Sources and calculation rules

  • Uninterrupted illness with an employer is the basic model. The worksheet shows the start of the weeks mentioned (first day of illness is day 1).
  • Employer and employee evaluate the reintegration in the meantime. Only the most important landmarks are included in this overview.
  • Wage sanction, early WIA, Sickness Benefits Act, maternity leave and combined periods of illness require separate assessment.

Frequently asked questions

Is week 104 always the last day of continued payment of wages?

Not always. For example, in the event of insufficient reintegration efforts, UWV can impose a wage sanction. Other exceptions and voluntary extensions also exist.

Can I write my diagnosis here?

Preferably use the worksheet for work agreements and document references. Medical information belongs to the company doctor and not in an overview that you may share with the employer.
