Applying for WAZO maternity benefit: timing, responsibility and documents

Notifying an employer about maternity leave is different from applying to UWV for maternity benefit. This guide explains the sequence, who handles each step and what to keep as evidence. Your employment or benefit position determines which application route to use.

Normally four to two weeks before leave starts. Notify your employer about the intended start at least three weeks beforehand.

Call UWV for the alternative route and the Aanvraag WAZO-uitkering form. Keep your earlier requests and do not wait only for HR.

The administrative application does not require your complete medical history. Discuss medical limitations with the appropriate professional.

1. Identify who should apply before selecting a form

For an employee, the employer normally submits the maternity benefit application. Someone receiving unemployment benefit uses Mijn UWV. For sickness benefit, an earnings-related WGA benefit and certain other situations, UWV directs applicants to call for the application form. Self-employed people investigate the separate ZEZ scheme. A form used by a colleague is therefore not necessarily suitable for your circumstances.

With more than one employer, identify the contact person and agreed leave start for each job. Keep separate confirmations so that an application for one employment is not mistaken for evidence covering the other. If you also work independently, ask UWV how the schemes interact. A cross-border or foreign-employer arrangement may require a separate insurance assessment; this guide does not determine which country's legislation applies to you.

2. Put three different timing rules on your calendar

Tell your employer your intended leave start at least three weeks beforehand. The usual UWV application window is four to two weeks before leave starts. UWV states that the decision is issued within four weeks after the start. These dates serve different purposes. A decision missing on your last working day does not, by itself, show that nobody applied.

Give every action an owner: your leave request, HR's submission and a check that UWV received it. Arrange the check before the final application week. If you change the intended start, ask who will report that change and whether an application has already been sent. Updating a team calendar while leaving a different date in the UWV application creates an avoidable discrepancy. Keep both systems aligned and request confirmation.

3. Collect the necessary information and limit medical disclosure

The expected delivery date comes from the pregnancy declaration issued by a doctor or midwife. Check that date against the chosen start and your current employment details. Tell the administrative contact if a fixed-term contract will expire during the benefit period. The recipient of the benefit and your bank details may then need special attention. Keep copies and record when each document was provided.

An administrative benefit application is not a request for your entire medical history. Health limitations may need assessment by the occupational physician, which is a separate process from payroll administration. Ask which information is needed, for what purpose and who will receive it. Use the appropriate secure channel. Avoid putting a diagnosis in a general scheduling system just because a leave request and a sickness discussion happen at the same time.

4. Verify receipt, dates and the payment recipient

Ask HR to confirm submission, the reported start date and who will receive the UWV payments. Employees commonly receive money through their employer, although direct payment can apply. An accurate daily rate is not the only issue: an incorrect recipient or start date can still disrupt your income planning. Check the administrative details before focusing exclusively on the expected amount.

Mijn UWV shows the application status. Save the confirmation of receipt and the later decision separately. An internal status marked completed is not a UWV receipt. If a date is wrong, report the correct date with supporting evidence and ask who will verify the correction. A forwarded message is evidence of a request, not evidence that the receiving system has actually changed. Keep the final confirmation as well.

Illustrative example

An employer confirms that leave is in the system. The employee separately asks for UWV receipt and discovers that only the staffing calendar was updated. HR can now submit the actual application using the agreed start. This fictional example distinguishes two administrative steps.

5. Act if your employer cannot or will not apply

UWV specifically provides a route where the employer cannot or will not make the application: call UWV and ask for the Aanvraag WAZO-uitkering form. Do not rely only on repeated messages to an absent manager. Explain the intended start, what has already been requested and which documents you hold. Ask what you can submit yourself to move the application forward.

Keep securing the benefit separate from resolving any dispute with the employer. Both can proceed alongside each other. Describe events factually without assuming that a legal breach has already been established. If leave starts soon, explain that urgency and record any agreed callback or next action. A complaint about an employer is not a substitute for a benefit application. Seek legal help separately where the refusal creates an employment dispute.

6. Read the decision beyond the approval sentence

Compare the start, duration, daily wage and payment route with your records. Direct recipients should also see the first payment date in the decision. A later birth can lead to a new decision setting the final end date. Retain earlier versions so you can identify what changed and which period a correction should cover. Do not throw away the initial award simply because a replacement arrives.

If something is wrong, ask which data were used and how to request correction. At the same time, protect the formal deadline stated in the decision. A telephone explanation or informal correction request does not automatically extend it. Use [checking the benefit amount](guide:hoogte-zwangerschapsuitkering) for calculation questions and [payment problems](guide:uitbetaling-zwangerschapsuitkering) if the award appears correct but the money has not arrived.

7. Track changes and later applications separately

An expiring contract, sickness before leave and the actual delivery date can affect administration. Report a relevant change to the organisation responsible for processing it. Ask whether HR is updating only its own records or also notifying UWV. After a telephone conversation, send a short factual confirmation recording the date, change and agreed next step. Keep the response with the original application.

Prepare a separate record for the final leave date, a baby's hospital admission and any flexible or parental leave arrangement. These do not necessarily use the same application. For each scheme, check whether a new request or a change to an existing award is needed. The guides on [contract expiry](guide:zwanger-contract-eindigt) and [leave duration](guide:duur-bevallingsverlof) explain what information to collect for those later steps.

Notify your employer of the intended start at least three weeks beforehand.

Check who applies to UWV in the four-to-two-week window.

Keep the dates submitted, receipt and chosen payment recipient.

Check the decision and record its formal challenge deadline.

When does an employer apply?

What if my employer refuses?

Call UWV for the alternative route and the Aanvraag WAZO-uitkering form. Keep your earlier requests and do not wait only for HR.

Must I give HR a diagnosis?

The administrative application does not require your complete medical history. Discuss medical limitations with the appropriate professional.

When will UWV decide?

UWV states within four weeks after leave starts. Follow the application status and protect the deadline in any decision you wish to challenge.
